You just write a letter of intent
state the address of the property, legal desctiption and your offer and when you can close and where. provide proof of funds letter too.
that’s it
there is no form
if you feel the need to justify your price, you can do that too.
if this is a listed property then you would make your offer through the listing agent. If you try to go around them and go directly to the lender, the lender will just tell you to contact their agent.
